Functional Movement 4 Life , HIGH GROUND JIU-JITSU WAIVER AND RELEASE OF LIABILITY

Location: IN8Life Health Complex – 1 Technology Lane, Murrysville, PA 15668

By signing this waiver, I acknowledge and agree to the following:

1. Voluntary Participation

I am voluntarily participating in Brazilian Jiu-Jitsu training and related physical activities offered by High Ground Jiu-Jitsu at IN8Life Health Complex.

2. Assumption of Risk

I understand that Jiu-Jitsu is a physically demanding activity with risks of injury, including but not limited to sprains, fractures, concussions, and other serious injuries. I knowingly and voluntarily assume full responsibility for these risks.

3. Physical Condition

I affirm that I am in good physical health and have no conditions that would prevent me from safely participating. I have consulted with a physician if needed.

4. Waiver and Release

I hereby release and hold harmless High Ground Jiu-Jitsu, IN8Life Health Complex, its owners, instructors, employees, and affiliates from any liability for personal injury, property damage, or loss, including claims caused by negligence, arising from my participation.

5. Medical Treatment

In the event of an injury, I authorize High Ground Jiu-Jitsu and its staff to seek medical treatment on my behalf and understand that I am responsible for any associated costs.

6. Rules and Conduct

I agree to follow all rules, instructions, and safety protocols provided by instructors. I understand that failure to do so may result in suspension or termination of participation.

7. Media Release (Optional)

I grant permission for High Ground Jiu-Jitsu to use photos or videos of me for promotional purposes.
